I talked to guys who were there after it happened. Low water, the cut off was full of scale. One guy had pictures on his cell phone, the top of the furnace was ripped open (downward) the rear door blew off and the boiler flew forward right through the wall. The boiler guy just made his rounds 5 min. earlier (I guess he did't check it too good) You must blow down the low water cut off at least once a day (we do ours once a shift) and if it does'nt shut off it needs to be taken out of service and repaired.
