Up here in Ontario, Canada, we have what are called "Guarded Plants". These are plants with so many safeties and interlocks that they interfere with each other.

Building owners and corporations love them because they feel they don't have to pay for an onsite engineer. The bean counters soon learn that calling a technition in every couple of days costs alot of money.

There is a mentality that if you have multiple and redundant safeties and interlocks you are OK, but no one takes into account that if a boiler locks out or shuts down, the root problem must be rectified.
