Need a lower door w/hardware

I am looking for the lower clean out door for a Old National-US Steel boiler, built by National US Radiator Corp in johnstown, PA
the boiler number is U3955

it was a oil fired boiler at one time and has been converted. The opening is 11" X 15 ".

I would like a door and the hardware to mount it.
