Faulty low water cut-outs, would be my guess.

Possibly followed by poor waterside maintaince. Lotta folks don't realize that just a thin layer of scale on a tube can cause a hot spot to develop, resulting in tube failure.

Still, according to the National Board, more boiler accidents are due to low water problems than any other cause. Operators should check that low water cut-out every shift!
