It's not very often that the problem lies with the safety or relief valve itself. It's most often with whatever the valve protects. The valve is usually just doing it's job.

If you've got a relief valve in liquid service, then I'd suspect a flooded expansion tank, or if it's a domestic water heater, that a back flow prevention device has been installed.
