I don't think you have any major problems. Given the age of the boiler and the tube material, which is plain old carbon steel, 4 plugged tubes doesn't seem to bad to me. During the next shutdown, randomly RT the tubes or even scope the inside of the tubes. Check for thickness. If the thickness has diminished 20% or more think about re-tubing. Obviously, as more tubes become plugged this is an indication that re-tubing is probably your best option.
