Gross incomplete combustion upon entry of the fuel into the furnace (fire chamber), then mixing of the remaining unburned fuel with secondary combustion air will cause a puffing explosion within the furnace combustion chamber that can eventually knock masonry walls down or bulge sheetmetal walls.
Surging within a boiler pressure vessel is another question.
