Make sure that the source of the hot condensate is functioning properly. I've seen people spend a lot of money to capture flash steam for other uses, only to discover that some traps get repaired elsewhere in the plant, and most of the flash steam disappears. If your condensate tank is just made of ordinary mild steel, you'll likely also discover that it's suffering from oxygen pitting due to un-deaerated make-up water being fed into it. The hot condensate will serve to drive the oxygen out of solution, and it will attack the steel tank.
